Base rate of $2,000 + care level $6,000 = $8,000*

Birchwood Manor
Base rate of $2,000 + care level $5,500 = $7,500*

Birchwood Living
Base rate of $2,000 + care level $5,500 = $7,500*

Birchwood Retreat
Base rate of $2,000 + care level $5,500 = $7,500*

Final rates are determined after a nurse evaluation.*

Resident Advocacy Services
(These services are offered in additional to the rates above.)

These items help to ease the family’s minds in the event of an emergency situation or when family members are unable to be present. A person who the resident is familiar with will be with your loved one.
__ Having our nurse accompany resident to the hospital in an emergency situation. - $125.00
__ Additional hours needed after check in. - $35.00 per hour
__ Hospital visit and conferring with a doctor or nurse after resident is taken to the emergency room. - $50.00
__ Accompanying resident to doctor visit outside of the home. - $75.00
__ Medication pick up from the pharmacy. - $25.00
__ Accompanying resident to any physical therapy. - $50.00
__ Any other services outside of the home requiring nurse attendance. - $35.00 per hour
